AVVACare supports anyone eligible for GUIDE – Guiding an Improved Dementia Experience, a new Center for Medicare & Medicaid Innovation (CMMI) program. GUIDE is available at $0 copay to individuals with dementia who are enrolled in traditional Medicare (Parts A and B), including those living in assisted living facilities. A confirmed dementia diagnosis is not required to begin the process.
Please note: Patients on hospice, living in long-term nursing home facilities, or enrolled in a Medicare Advantage plan or PACE program are not eligible for GUIDE.

After you complete our contact form, our Care Navigator team will reach out to schedule an appointment with one of our Dementia Care Specialists. We’ll confirm your eligibility for Medicare’s GUIDE program, complete a comprehensive assessment, and handle all the paperwork for Medicare submission.
Once Medicare approves your enrollment, we’ll introduce you to your dedicated care team – including your AVVACare Navigator – who will work with you to build a personalized dementia care plan.
Your AVVACare Navigator will be your main point of contact, available 24/7 to answer questions, coordinate care, arrange respite and community resources, and support you and your family every step of the way.

AVVACare is a team of physicians and dementia care specialists passionate about supporting families living with dementia. When Medicare launched the GUIDE program to provide much-needed services for patients and caregivers, we saw an opportunity – but also a gap: many families weren’t connected to providers participating in the program.
That’s why we created AVVACare – to make GUIDE’s benefits accessible to patients and caregivers nationwide, no matter who their current doctor is. Our approach is compassionate and holistic, focused on easing the caregiving journey and improving quality of life for both patients and their loved ones.

The GUIDE Model is a new care delivery program from the Centers for Medicare & Medicaid Innovation (CMMI), created to improve support for people living with dementia and their caregivers. It takes a whole-person approach – combining expert care coordination, caregiver support, and access to community-based services that meet the unique needs of each family.
GUIDE emphasizes close coordination between medical providers and non-clinical services, helping families navigate the challenges of dementia more effectively, improve care outcomes, and ease the burden on caregivers.

No – AVVACare is completely free for individuals and families who qualify through Medicare’s GUIDE program, created by the Centers for Medicare & Medicaid Innovation (CMMI).
To be eligible, individuals must:
Have a dementia diagnosis
Be enrolled in traditional Medicare (Parts A and B)
Not be enrolled in a Medicare Advantage plan or PACE
Not have elected hospice
Not be living in a long-term nursing home facility

No, you will not lose your existing coverage. GUIDE is designed to complement and wrap around the services you already receive. Our AVVACare Care Navigators can help you coordinate and manage all your available resources to ensure you get the most comprehensive support possible.
